ABSTRACT

The aim of this study was to improve our knowledge about carcinogenesis and lifestyle, given their impact on the occurrence of breast cancer, emphasizing the importance of lifestyle changes as a preventive factor in the development of the disease. We conducted a bibliographic review with the analysis of 31 articles in English and Portuguese. As a result, the articles selected for study showed that factors such as diet, alcohol intake, smoking, obesity, physical activity, occupational exposure, hormonal factors (hormone therapy, contraceptives) and reproductive factors (menarche, menopause, nulliparity, pregnancy, breastfeeding) have a protective or risk effect on breast cancer. We conclude that eating healthy, with fruits, vegetables and greens, practicing moderate physical activity, avoiding alcoholic beverages and breastfeeding exclusively reduce the risk of developing breast cancer by 28%. Therefore, it is necessary to make the public aware of these modifiable risk factors

Introducción. El cáncer de mama es el tipo de cáncer que genera más muertes en mujeres en el mundo. Aunque se reconoce el aporte de factores genéticos, hormonales y de estilos de vida como sus principales causas, las hipótesis que señalan que la contaminación del ambiente juega un papel importante en su desarrollo, han tomado mucha fuerza en los últimos años. Estas hipótesis surgen debido a que el aumento en la incidencia del cáncer de mama coincide con procesos de industrialización, además de mayor presencia en regiones urbanas y con altos niveles de contaminación. El objetivo de este artículo fue consolidar información sobre los mecanismos fisiopatológicos que puedan explicar la relación entre cáncer de mama y la contaminación por material particulado. Metodología. Se realizó una búsqueda de literatura en PubMed, Google Académico y Epistemonikos para documentos publicados sobre el tema desde enero de 2016 hasta el 3 de agosto de 2022. Resultados. Se encontró que algunos de los mecanismos que podrían explicar dicha relación incluyen: alteraciones endocrinas que favorecen cambios hormonales, induciendo el crecimiento mamario; cambios en las características histológicas del tejido normal, como involución reducida de unidades lobulares ductales terminales; formación de aductos de hidrocarburos aromáticos policíclicos-ácido desoxirribonucleico (HAP-ADN), con mutación específica del gen TP53; activación de la proliferación en la línea celular MCF-7; y, alteraciones en la metilación del ADN. Conclusión. Si bien órganos distales como la mama no son la primera entrada de los contaminantes ambientales al cuerpo, estos sí pueden verse afectados tras la exposición a largo plazo, a través de diferentes mecanismos de disrupción endocrina y daño al ADN principalmente


Breast cancer is the type of cancer that causes the most deaths in women worldwide. Although the contribution of genetic, hormonal and lifestyle factors are recognized as its main causes, the hypotheses that indicate that environmental pollution has an important role in its development have taken on great strength during the last years. These hypotheses are based on the increase in the incidence of breast cancer that coincides with industrialization processes, in addition to its greater presence in urban regions with high levels of pollution. The aim of this study was to consolidate information on the pathophysiological mechanisms that can explain the relationship between breast cancer and air pollution by particulate matter. Methodology. A literature search was carried out in PubMed, Google Scholar and Epistemonikos for documents published on this topic from January 2016 until August 3rd 2022. Results. Some of the mechanisms that could explain this association include endocrine alterations that favor hormonal changes, inducing breast growth; changes in the histological characteristics of normal tissue such as reduced involution of terminal duct lobular units; formation of polycyclic aromatic hydrocarbons-deoxyribonucleic acid (PAH-DNA) adducts, with specific mutation of the TP53 gene; an increase in cell proliferation in the MCF-7 cell line; and alterations in DNA methylation. Conclusion. Although distal organs such as the breast are not the entry site of environmental pollutants into the body, they can be affected after prolonged exposure, mainly through different mechanisms of endocrine disruption and DNA damage

Introducción: La mastectomía es una transformación dolorosa, con diversas repercusiones emocionales. La mujer que se enfrenta a este proceso a consecuencia del cáncer, se encuentra en una posición difícil, con pocas opciones, al considerar las posibilidades de continuar con su existencia y los cambios que esto implica. Objetivo: Conocer los cambios de estilo de vida que experimentan las mujeres con cáncer de mama después de la mastectomía. Métodos: Estudio cualitativo con diseño interpretativo, realizado en la Unidad de Oncología del Hospital Regional de Lambayeque, Chiclayo, Perú, de enero a junio de 2021. Por muestreo no probabilístico, fueron seleccionadas seis mujeres mastectomizadas, mayores de 18 años, con seguimiento en consulta mayor de un año y un familiar conviviente mayor de 18 años. Los datos se obtuvieron mediante entrevista semiestructurada y fueron analizados según el modelo de adaptación de Callista Roy. Resultados: La información recogida se categorizó, en correspondencia a cada elemento del modelo de adaptación de Callista Roy, en: necesidad fisiológica, cambios en la alimentación, dominio de rol, cambios en el rol de madre y esposa, y en interdependencia: las relaciones con los que la rodean (amigos, familia) y el aspecto laboral-profesional. Se destacaron cambios drásticos en la selección de alimentos, aislamiento, reducción de actividades; efecto en la carga laboral; importancia del apoyo familiar. Conclusión: El cáncer y la mastectomía exigen adaptaciones en la paciente y la familia. Sin embargo, ocurren transformaciones afectivas, que mueven sentimientos, valoración, admiración y acercamiento, que refuerzan los vínculos de pareja, familiares y de amigos, tan necesarios para lidiar con el proceso(AU)


Introduction: Mastectomy is a painful transformation, with diverse emotional repercussions. The woman who faces this process as a consequence of cancer finds herself in a difficult position, with few options, when considering the possibilities of continuing with her existence and the changes this implies. Objective: To know the lifestyle changes experienced by women with breast cancer after mastectomy. Methods: Qualitative study with interpretative design, conducted in the Oncology Unit of the Regional Hospital of Lambayeque, Chiclayo, Peru, from January to June 2021. By non-probabilistic sampling, six mastectomized women, older than 18 years, with a follow-up in consultation of more than one year and a cohabiting relative older than 18 years were selected. The data were obtained by semi-structured interview and were analyzed according to Callista Roy's adaptation model. Results: The information collected was categorized, according to each element of Callista Roy's adaptation model, as follows: physiological need, changes in diet, role dominance, changes in the role of mother and wife, and interdependence: relationships with those around her (friends, family) and the work-professional aspect. Drastic changes in food selection, isolation, reduction of activities; effect on workload; importance of family support were highlighted. Conclusion: Cancer and mastectomy require adaptations in the patient and family. However, emotional transformations occur, which move feelings, appreciation, admiration and closeness, which reinforce the couple, family and friends bonds, so necessary to deal with the process(AU)

Más de nueve millones de sobrevivientes de cáncer de mama en todo el mundo sufren un deterioro en la calidad de vida atribuible a los síntomas de la menopausia relacionados con el déficit de los estrógenos y con los efectos secundarios de la terapia contra el cáncer. La terapia de reemplazo hormonal (TRH) es muy eficaz para controlar estos síntomas en la población general y en las sobrevivientes de cáncer de mama. Sin embargo, la preocupación de la recurrencia del cáncer de mama como resultado del uso de TRH impide que muchos oncólogos utilicen este enfoque en el tratamiento de los síntomas menopáusicos. La evidencia de ensayos aleatorizados, estudios observacionales y meta-nálisis sobre el impacto del uso de TRH en la recurrencia y supervivencia del cáncer de mama sigue siendo controvertida.


More than nine million breast cancer survivors in everyone suffers a deterioration in the quality of life attributable to the symptoms of menopause related to the deficiency of the estrogens and with the side effects of anti-estrogen therapy Cancer. Hormone replacement therapy (HRT) is very effective in controlling these symptoms in the general population and in survivors of breast cancer. Abstract Objective To evaluate the distribution of the main sociodemographic and clinicalpathological characteristics in women with breast cancer according to the molecular profile by immunohistochemistry. Methods A cross-sectional, retrospective, analytical and quantitative study was performed, with an analysis of 137 medical records from January 2015 to December 2018 of women attending the High Complexity in Oncology Unit of the city of Imperatriz, state of Maranhão, Brazil. The immunohistochemical profile of tumors based on the estrogen and progesterone receptor, Human Epidermal growth factor Receptor-type 2 (HER2) overexpression and Ki67 cell proliferation indexwas defined, fromwhich six molecular subtypes were determined: luminal A, luminal B-HER2 negative, luminal B-HER2 positive, triple negative, overexpression of HER2 and inconclusive. Results A total of 52.6% of the patients were postmenopausal, mean age 52.1 years old, brown (56.2%), had a schooling level < 9 years (40%), staging > IIB (52.6%) and 23.4% hadmetastasis. Invasive ductal carcinoma accounted for 84.7%, tumor size was 2 to 5 cm (48.9%), with lymph node involvement (56.2%), axillary lymphadenectomy in 67.2%, andmastectomy in 73.7% of the patients. Themost frequentmolecular subtype was the luminal B-HER2 negative (36.5%), and the luminal A subtype showed characteristics of better prognosis when compared with the others. Conclusion It was concluded that in the association of molecular subtypes with sociodemographic and clinical-pathological characteristics, there were no statistically significant results obtained, except for complementary therapy, referring to hormone therapy, and there was a high index of metastasis at diagnosis, which was a worrying factor and indicative of failures in the screening and early diagnosis of this population.


Resumo Objetivo Avaliar a distribuição das principais características sociodemográficas e clínico-patológicas em mulheres com câncer de mama segundo o perfil molecular pela imunohistoquímica. Métodos Estudo transversal, retrospectivo, analítico, descritivo e quantitativo, com análise de 137 prontuários do período de janeiro de 2015 a dezembro de 2018 de mulheres atendidas na Unidade de Assistência da Alta Complexidade em Oncologia da cidade de Imperatriz, MA, Brasil. Foi definido o perfil imunohistoquímico dos tumores baseado na avaliação dos receptores de estrogênio e progesterona, superexpressão de HER2 e índice de proliferação celular Ki67, de onde foram determinados seis subtipos moleculares: luminal A, luminal B-HER2 negativo, luminal B-HER2 positivo, triplo negativo, superexpressão de HER2 e inconclusivo. Resultados Foi demonstrado que 52,6% das pacientes eram pós-menopausadas, com idademédia de 52,1 anos, pardas (56,2%), tinhamgrau de escolaridade < 9 anos (40%), estadiamento > IIB (52,6%) e 23,4% tinham metástase. Carcinoma ductal invasivo representou 84,7%, o tamanho tumoral foi de 2 a 5 cm (48,9%), com comprometimento linfonodal (56,2%), com linfadenectomia axilar em 67,2% e mastectomia em 73,7% das pacientes. O subtipo molecularmais frequente foi o luminal B-HER2 negativo (36,5%), e o subtipo luminal A apresentou características de melhor prognóstico em relação aos demais. Conclusão Concluiu-se que na associação dos subtipos moleculares com as características sociodemográficas e clínico-patológicas não se obteve resultados com significância estatística, exceto para terapia complementar, referente à hormonioterapia, e houve elevado índice de metástase ao diagnóstico, o que representou um fator preocupante e indicativo de falhas no rastreio e diagnóstico precoce dessa população.

ABSTRACT BACKGROUND: The largest radiological accident to occur in any urban area happened in Goiânia, Brazil, in 1987. OBJECTIVE: To evaluate the association between breast cancer incidence and ionizing radiation levels. DESIGN AND SETTING: Ecological study among residents of the city of Goiânia, Brazil. METHODS: The central region of Goiânia, with seven major sources of contamination from cesium-137, was defined as the study area. The addresses of women diagnosed with breast cancer were identified between 2001 and 2010. The data were geographically referenced and, using census data, the annual averages of crude incidence rates were estimated. The existence of clusters of new cases was ascertained by means of the Moran index. Correlations of radiometric measurements with the incidence were assessed using unconditional linear regression. RESULTS: A total of 4,105 new cases were identified, of which 2,233 were in the study area, and of these, 1,286 (57.59%) were georeferenced. The gross rates of total and referenced cases were 102.91 and 71.86/100,000 women, respectively. These were close to the average for Brazilian state capitals, which is 79.37/100,000 women. The cluster analysis showed slight correlations in three small sets of census tracts, but these were far from the sources of contamination. The scatter plot of points and the R2 value close to zero indicated that there was no association between the variables. CONCLUSION: This study reinforces the hypothesis that the ionizing radiation levels to which women living in Goiânia are now exposed to are not associated with the onset of new cases of breast cancer.

Resumen El cáncer de mama es uno de los tipos más frecuentes de cáncer en la población femenina y presenta una estrecha relación con la herencia genética. El asesoramiento genético procura informar y guiar a las pacientes durante todo el proceso de identificación y diagnóstico de cáncer de mama hereditario. Por lo que con el presente artículo se pretende demostrar la relevancia del asesoramiento genético en el abordaje de las pacientes con cáncer de mama hereditario. Se realizó una revisión sistemática de la literatura para la selección de publicaciones científicas que aporten conocimiento sobre el cáncer de mama en relación con la herencia genética y otros documentos que demuestren la importancia del asesoramiento genético en pacientes con cáncer de mama hereditario. Con esta revisión, se obtuvo información valiosa sobre la importancia de la implementación del asesoramiento genético, durante la realización de pruebas genéticas, en pacientes con cáncer de mama hereditario.


Abstract Breast cancer is one of the most frequent types of cancer in the female population and has a close relationship with genetic inheritance. Genetic counseling seeks to inform and guide patients throughout the process of identification and diagnosis of hereditary breast cancer. Therefore, this article intends to demonstrate the relevance of genetic counseling in the approach of patients with hereditary breast cancer. It has been made a systematic review of the literature for the selection of scientific publications that provide knowledge about breast cancer in relation to genetic inheritance and other documents demonstrating the importance of genetic counseling in patients with hereditary breast cancer was performed. With this review, valuable information was obtained on the importance of the implementation of genetic counseling, during genetic testing, in patients with hereditary breast cancer.

ABSTRACT

Abstract: Objective: To describe the rationale and the methodology of a multicenter project to study the etiology of breast cancer in young Latin American women. Materials and methods: The International Agency for Research on Cancer has established an international collaborative population-based case-control study in four countries in Latin America: Chile, Colombia, Costa Rica, and Mexico (the PRECAMA study). Standardized methodologies were developed to collect information on reproductive variables, lifestyle, anthropometry, diet, clinical and pathological data, and biological specimens. The study will be extended to other countries in the region. Conclusion: PRECAMA is unique in its multidisciplinary approach that combines genetics, genomics, and metabolomics with lifestyle factors. The data generated through this project will be instrumental to identify major risk factors for molecular subtypes of breast cancer in young women, which will be important for prevention and targeted screening programs in Latin America.


Resumen: Objetivo: Describir la justificación y la metodología para el establecimiento de un proyecto multicéntrico sobre el cáncer de mama en mujeres jóvenes de América Latina. Material y métodos: La Agencia Internacional para la Investigación del Cáncer (IARC) ha establecido un estudio colaborativo internacional de casos y controles con base poblacional en cuatro países de América Latina: Chile, Colombia, Costa Rica y México (el estudio PRECAMA). Se han desarrollado metodologías estandarizadas para recolectar información sobre variables reproductivas, estilos de vida, antropometría y dieta, datos clínicos y patológicos y muestras biológicas. Conclusión: PRECAMA es único en su enfoque multidisciplinario. Los datos generados a través de este proyecto serán fundamentales para identificar los principales factores de riesgo del cáncer de mama en mujeres jóvenes. Los hallazgos serán relevantes para la prevención y los programas de detección oportuna en América Latina, con beneficios clínicos inmediatos.

SUMMARY Breast Cancer is common in women, but its etiology is not yet fully understood. Several factors may contribute to its genesis, such as genetics, lifestyle, and the environment. Melatonin may be involved in the process of breast cancer. Therefore, the aim of this study is to evaluate the influence of the levels of melatonin on breast cancer through a systematic review and meta-analysis. We performed a systematic review according to PRISMA recommendations. The primary databases MEDLINE, Embase, and Cochrane were consulted. There was no restriction on the year of publication and language. Data of systematic reviews from April 2017 to September to 2017 were analyzed. The meta-analysis was conducted using RevMan 5.3 software provided by the Cochrane Collaboration. From a total of 570 articles, 9 manuscripts were included in this review. They analy onzed women with breast cancer and control patients, of which 10% and 90% were in the reproductive period and after menopause, respectively. The lowest level of melatonin was found in approximately 55% of studies with breast cancer in post-menopause. The metanalyses of the studies demonstrated low levels of melatonin in breast cancer patients (n=963) compared with control patients (n= 1332), with a mean difference between the studies of 8722;3.54 (CI8722;6.01,8722;1.06). Another difference found was in the comparison between smoking patients, with an average difference between 1.80 [0.97-2.63]. Our data suggest that low levels of melatonin might be a risk factor for breast cancer.


RESUMO O câncer de mama é comum em mulheres, mas sua etiologia ainda não é totalmente compreendida. Vários fatores podem contribuir para sua gênese, genética, estilo de vida e meio ambiente. A melatonina pode estar envolvida no processo de câncer de mama. Portanto, o objetivo deste estudo é avaliar a influência dos níveis de melatonina no câncer de mama por meio de uma revisão sistemática e meta-análise. Realizamos uma revisão sistemática de acordo com as recomendações do Prisma. Os principais bancos de dados, Medline, Embase e Cochrane, foram consultados. Não houve restrição quanto ao ano de publicação e idioma. Os dados de revisão sistemática obtidos de abril de 2017 a setembro a 2017 foram analisados. A meta-análise foi conduzida pelo programa RevMan 5.3 fornecido pela Cochrane Collaboration. De um total de 570 artigos, nove foram incluídos nesta revisão. As análises foram conduzidas em mulheres com câncer de mama e pacientes controle, dos quais 10% e 90% estavam no período reprodutivo e após a menopausa, respectivamente. O nível mais baixo de melatonina foi encontrado em aproximadamente 55% dos estudos com câncer de mama na pós-menopausa. As meta-análises de estudos demonstraram os baixos níveis de melatonina em doentes com câncer da mama (n=963), em comparação com os pacientes de controle (n=1.332), sendo a diferença de médias entre os estudos da 8722;3,54 (CI 8722;6,01, 8722;1,06). Outra diferença é demonstrada nas comparações entre pacientes fumantes, sendo a diferença da média entre 1,80 [0,97-2,63]. Nossos dados sugerem que baixos níveis de melatonina podem ser um fator de risco para câncer de mama.

Abstract Objective Epidemiological studies have shown evidence of the effect of sex hormones in the pathogenesis of breast cancer, and have suggested a relationship of the disease with variations in genes involved in estrogen synthesis and/or metabolism. The aim of the present study was to evaluate the association between the CYP3A4*1B gene polymorphism (rs2740574) and the risk of developing breast cancer. Methods In the present case-control study, the frequency of the CYP3A4*1B gene polymorphism was determined in 148 women with breast cancer and in 245 women without the disease. The DNA of the participants was extracted from plasma samples, and the gene was amplified by polymerase chain reaction. The presence of the polymorphism was determined using restriction enzymes. Results After adjusting for confounding variables, we have found that the polymorphism was not associated with the occurrence of breast cancer (odds ratio = 1.151; 95% confidence interval: 0.714-1.856; p= 0.564). We have also found no association with the presence of hormone receptors, with human epidermal growth factor receptor 2 (HER2) overexpression, or with the rate of tumor cell proliferation. Conclusion We have not observed a relationship between the CYP3A4*1B gene polymorphism and the occurrence of breast cancer.


Resumo Objetivo Estudos epidemiológicos têm mostrado evidências da influência dos hormônios sexuais na patogênese do câncer de mama, e têm sugerido uma relação entre a doença e variações em genes envolvidos na síntese e/ou metabolização de estrógenos. O objetivo do presente estudo foi avaliar a associação entre o polimorfismo do gene CYP3A4*1B (rs2740574) e o risco de desenvolvimento da neoplasia mamária. Métodos No presente estudo de caso-controle, a frequência de polimorfismo do gene CYP3A4*1B foi determinada em 148 mulheres com câncer de mama, e em 245 mulheres sem a doença. O DNA das participantes foi extraído do plasma, e o gene foi amplificado por meio de reação em cadeia da polimerase, enquanto o polimorfismo foi determinado por enzimas de restrição. Resultados O polimorfismo, após o ajuste para variáveis de confusão, não foi associado à ocorrência de câncer de mama (razão de possibilidades = 1,151; intervalo de confiança de 95%: 0,714-1,856; p= 0,564). Também não observamos associação com a presença de receptores hormonais, superexpressão do receptor tipo 2 do fator de crescimento epidérmico humano (HER2, na sigla em inglês), ou com a taxa de proliferação celular do tumor. Conclusão Não observamos relação entre o polimorfismo do gene CYP3A4*1B e a ocorrência de câncer de mama.

ABSTRACT Objective: This study was conducted to assess the serum 25-hydroxy (OH) vitamin D levels in patients with breast cancer compared to healthy controls and to identify its association with aggressive breast cancer phenotypes. Materials and methods: Serum 25-OH vitamin D levels of 78 breast cancer patients and 78 matched healthy controls were estimated using ELISA. The cases and controls were matched with respect to age, menopausal status, parity, weight, height and co-morbidities. Prognostic factors like grade of tumour, hormone receptor status, HER2 neu status and lymphovascular invasion were compared with 25-OH vitamin D levels. Results: The mean serum 25-OH vitamin D levels of cases were significantly lower compared to the controls (22.33 ± 8.19 vs. 37.41 ± 12.9 ng/mL; p = 0.0001). Patients with higher grades of tumour, non-luminal types of breast cancer and breast cancers with estrogen receptor negativity had significantly lower serum 25-OH vitamin D levels than their opposing groups. Patients with excellent and good Nottingham's prognostic Index (NPI) had significantly higher serum 25-OH vitamin D levels than the moderate and poor NPI groups. Conclusion: Newly diagnosed breast cancer patients have significantly lower serum 25-OH vitamin D levels than healthy controls. Lower level of serum 25-OH vitamin D correlates with aggressive breast cancer phenotypes.

ABSTRACT

SUMMARY Cancer is characterized by the disordered growth of cells that have high capacity of invasion to the tissues and organs. One of the types of tumour that has national incidence and high mortality is breast cancer. Studies show that in addition to hereditary factors, lifestyle and environmental factors, there are factors related to emotional distress (mourning), which interfere with the development of breast cancer. Thus, it is necessary to investigate if the experience of mourning can trigger the appearance of the tumour. For this, an integrative review was performed to verify the existence of the relationship between mourning and development of breast cancer, which presented contradictory results. Methodological errors and lack of access to important information, such as alcohol and tobacco use, were pointed out as the main causes of the contradiction found. A possible mechanism involving cortisol release has been proposed, but more research is needed to make it clear whether the association between mourning and breast cancer really exists, and by what path.


RESUMO: O câncer é caracterizado pelo crescimento desordenado das células que possuem alta capacidade de invasão aos tecidos e órgãos. Um dos tipos de tumour que possui incidência nacional e alta mortalidade é o câncer de mama. Estudos mostram que, além dos fatores hereditários, ambientais e dos hábitos de vida, existem fatores relacionados a um trauma emocional (luto) que interferem no desenvolvimento do câncer de mama. Dessa forma, é necessário investigar se a vivência do luto pode desencadear o aparecimento do tumour. Para isso, realizou-se uma revisão integrativa para verificar a existência da relação entre o luto e o desenvolvimento do câncer de mama, que apresentou resultados contraditórios. Os erros metodológicos e a falta de acesso a informações importantes, como uso de álcool e fumo, foram apontadas como as principais causas da contradição encontrada. Um possível mecanismo envolvendo liberação de cortisol tem sido proposto, mas são necessárias mais investigações para que fique claro se a associação entre luto e câncer de mama realmente existe, e por qual mecanismo ocorre.

ABSTRACT

In Chile breast cancer (BC) is the first cause of death in women. While the most important risk factor for its development is estrogenic stimulation, environmental factors and lifestyles also contribute to its pathogenesis. Epidemiological studies show a direct relationship between physical activity (PA), incidence and recurrence of BC. Supervised PA practice is recommended in most cancer patients to improve their quality of life, to reduce adverse effects from treatment and eventually to improve the prognosis of the disease. We review the epidemiological evidence linking PA and BC and the biological basis of this relationship. We also review the relevant interventional studies and we explore some practical indications of PA in patients with BC, as a model for other tumors of epidemiological importance.

Summary Zinc is the catalytic component of proteins that regulate responses to DNA damage, intracellular signaling enzymes, and matrix metalloproteinases, which are important proteins in carcinogenesis. The objective of this review is to bring current information on the participation of zinc and matrix metalloproteinases types 2 and 9 in mechanisms involved in the pathogenesis of breast cancer. We conducted a literature review, in consultation with the PubMed, Lilacs, and Scielo databases. The zinc and cysteine residues are structural elements shared by all members of the family of matrix metalloproteinases, and these proteins appear to be involved in the propagation of various types of neoplasms, including breast cancer. Moreover, transported zinc is likely to be used for the metalation of the catalytic domain of the newly synthesized metalloproteinases before the latter are secreted. Accordingly, increase in zinc concentrations in cellular compartments and the reduction of this trace element in the blood of patients with breast cancer appear to alter the activity of metalloproteinases 2 and 9, contributing to the occurrence of malignancy. Thus, it is necessary to carry out further studies with a view to clarify the role of zinc and metalloproteinases 2 and 9 in the pathogenesis of breast cancer.


Resumo O zinco é componente catalítico de proteínas que regulam respostas a danos no DNA, enzimas de sinalização intracelular e metaloproteinases de matriz, proteínas importantes na carcinogênese. O objetivo desta revisão é trazer informações atualizadas sobre a participação do zinco e das metaloproteinases de matriz dos tipos 2 e 9 em mecanismos envolvidos na patogênese do câncer de mama. Realizou-se um levantamento bibliográfico, mediante consulta às bases de dados PubMed, Scielo e Lilacs. O zinco e os resíduos de cisteína são elementos estruturais compartilhados por todos os membros da família das metaloproteinases de matriz, as quais parecem estar envolvidas na propagação de vários tipos de neoplasias, incluindo o câncer de mama. Além disso, é provável que o zinco transportado seja utilizado para metalação do domínio catalítico das metaloproteinases recentemente sintetizadas antes de serem segregadas. Nesse sentido, o aumento das concentrações de zinco em compartimentos celulares e a redução desse oligoelemento no sangue de pacientes com câncer de mama parecem alterar a atividade das metaloproteinases 2 e 9, contribuindo para a ocorrência de tumor maligno. Assim, faz-se necessária a realização de novos estudos na perspectiva de esclarecer o papel do zinco e das metaloproteinases 2 e 9 na patogênese do câncer de mama.

Abstract The aim of this integrative review was to compare Latin American literature about risk and knowledge on breast cancer. Of 47 studies selected, 20 were about knowledge or awareness and 27 about risk of breast cancer. English was the dominant language in studies about risk, whereas studies about knowledge were mainly written in Spanish or Portuguese. Studies about knowledge were all cross- sectional, whereas case- control studies dominated authors’ interest about risk of breast cancer. Studies about knowledge were mainly focused on early detection of the disease and the most common study objective was breast self- examination (N = 14). In contrast, few studies about risk of breast cancer focused on early detection (N = 5). Obesity and overweight (N = 14), family history (N = 13), decreased parity (N = 12), and short breastfeeding duration (N = 10) were among the most frequent identified risk factors. Socio- economic factors such as income and educational level had variable effects on breast cancer risk and affected also knowledge of women about risk factors and early detection. Present results indicated that studies about risk of breast cancer were more often based on a better sound analytical background, compared to studies about knowledge, which were mostly descriptive.


Resumo Esta revisão teve por objetivo comparar a literatura Latino-Americana sobre o conhecimento da neoplasia mamária e seus fatores de risco. Foram selecionados 47 estudos, dos quais 27 eram sobre o risco de desenvolver câncer de mama e 20 sobre o conhecimento ou a consciência sobre a doença. Os estudos sobre fatores de risco eram principalmente do tipo caso-controle e sua maioria estava no idioma Inglês. Já as pesquisas sobre conhecimento eram, sobretudo, do tipo transversal e foram publicadas nos idiomas Português e Espanhol. As pesquisas sobre conhecimento destacaram o diagnóstico precoce da neoplasia mamária, e o autoexame da mama foi o método de detecção mais abordado (N = 14). Por outro lado, os estudos sobre fatores de risco avaliaram, principalmente, sobrepeso (N = 14), historia familiar (N = 13), baixa paridade (N = 12) e curto período de amamentação (N = 10). Fatores socioeconômicos, como a renda e o nível educacional tiveram efeitos variáveis e afetaram também o conhecimento das mulheres sobre fatores de risco e detecção precoce. Os resultados da pesquisa indicaram que os estudos sobre fatores de risco do câncer de mama apresentaram melhor delineamento metodológico e fundamentação teórica, comparado com os estudos sobre conhecimento que foram na maioria dos casos descritivos.

Introducción: los factores de riesgo convencionales solo pueden explicar una parte del cáncer de mama esporádico. Se ha propuesto que las infecciones causadas por el virus del tumor mamario en ratón, el virus de Epstein Barr y el virus del papiloma humano constituyen un factor de riesgo potencial para el desarrollo de esta enfermedad. Objetivo: realizar una actualización acerca del papel de algunas infecciones virales en el desarrollo del cáncer de mama. Métodos: se realizó una revisión de los artículos publicados sobre este tema en algunas bases de datos (PubMed, Medline, BioMed Central y SciELO). Conclusiones: aunque existen criterios controversiales sobre el tema, algunos resultados sugieren que la acción de estos virus puede contribuir al desarrollo de las enfermedades malignas de la mama(AU)


Introduction: conventional risk factors cannot explain all cases of sporadic breast cancer. Infections caused by the mouse mammary tumor virus, Epstein-Barr virus and human papillomavirus have been suggested to be potential risk factors for this condition. Objective: to update the information on the role of some viral infections in the development of breast cancer. Methods: a review was conducted of papers about the subject published in several databases (PubMed, Medline, BioMed Central and SciELO). Conclusions: despite the fact that criteria on the subject are controversial, some results suggest that the action of these viruses may contribute to the development of malignant breast disease(AU)

ABSTRACT

Abstract Causal attribution among women with breast cancer was studied. The study included 157 women outpatients with breast cancer. A form for sociodemographic and clinical data and the Revised Illness Perception Questionnaire (IPQ-R) were used. The results showed that women attributed breast cancer primarily to psychological causes, which does not correspond to known multifactorial causes validated by the scientific community. Providing high quality, patient-centered care requires sensitivity to breast cancer women’s beliefs about the causes of their cancer and awareness of how it can influence patient’s health behaviors after diagnosis. If women with breast cancer attribute the illness to modifiable factors then they can keep a healthy lifestyle, improving their recovery and decrease the probability of cancer recurrence after diagnosis. Lymphedema secondary to breast cancer causes physical and psychological morbidity and compromises quality of life. The objective of this literature review was to study lymphatic compensation after surgery for breast cancer and the factors that influence this process, with a view to understanding the etiopathogenesis of lymphedema. Articles indexed on Pubmed published from 1985 to 2012 were reviewed. According to the literature, lymphangiogenesis reduces damage to lymph vessels; there is little evidence that Vascular Endothelial Growth Factor is elevated in women with lymphedema; lymphovenous communications can be observed 60 days after surgery; women without lymphedema have acquired alternative mechanisms for removal of proteins from the interstitial space; and active exercise stimulates lymphatic and venous pumping. Health professionals should teach these patients about the risk factors for lymphedema. The effects of lymphangiogenesis, proteolysis and lymphovenous communications on development of lymphedema should be studied, since these events are intimately related.


Linfedema secundário ao câncer de mama resulta em morbidade física e psicológica, e compromete a qualidade de vida. O objetivo desta revisão da literatura foi estudar as compensações linfáticas após cirurgia para câncer de mama e os fatores que interferem neste processo, visando a compreender a etiopatogenia do linfedema. Foram incluídos artigos publicados de 1985 a 2012, da base de dados Pubmed. Segundo a literatura, a linfangiogênese reduz danos nos vasos linfáticos; há pouca evidência de que o fator de crescimento vascular endotelial linfático é aumentado nas mulheres com linfedema; anastomoses linfovenosas são observadas 60 dias após a cirurgia; mulheres sem linfedema adquiriram mecanismos alternativos para remoção das proteínas do interstício, e exercício ativo estimula bombeamento linfático e venoso. Orientações dos fatores de risco para linfedema devem ser empregadas por profissionais da saúde. O efeito de linfangiogênese, proteólise e anastomoses linfovenosas sobre o desenvolvimento do linfedema deve ser estudado, pois são eventos intimamente relacionados.

ABSTRACT

OBJECTIVE: to assess the clinical effect of topical treatment using Ulmo honey associated with oral ascorbic acid in patients with venous ulcers. METHOD: longitudinal and descriptive quantitative study. During one year, 18 patients were assessed who were clinically diagnosed with venous ulcer in different stages, male and female, adult, with a mean injury time of 13 months. Ulmo honey was topically applied daily. The dressing was applied in accordance with the technical standard for advanced dressings, combined with the daily oral consumptions of 500 mg of ascorbic acid. The monitoring instrument is the assessment table of venous ulcers. RESULTS: full healing was achieved in 100% of the venous ulcers. No signs of complications were observed, such as allergies or infection. CONCLUSION: the proposed treatment showed excellent clinical results for the healing of venous ulcers. The honey demonstrated debriding and non-adherent properties, was easy to apply and remove and was well accepted by the users. The described results generated a research line on chronic wound treatment. .


OBJETIVO: avaliar o efeito clínico de tratamento tópico com mel de Ulmo associado à administração oral de ácido ascórbico em pacientes portadores de úlceras venosas. MÉTODO: estudo quantitativo descritivo longitudinal. Um total de 18 pacientes adultos, ambos os sexos, clinicamente diagnosticados com úlcera venosa em diferentes estágios e com duração de 13 meses em média, foram avaliados pelo período de um ano. A aplicação tópica diária de mel de Ulmo foi realizada de acordo com a norma técnica de tratamento avançado combinada com o consumo diário de 500 mg de ácido ascórbico. O instrumento usado para monitoramento foi a tabela de avaliação de úlceras venosas. RESULTADOS: cicatrização completa foi observada em 100% das úlceras venosas. Não foram observados sinais de complicação tais como alergias ou infecção. CONCLUSÃO: o tratamento proposto apresentou resultados clínicos excelentes na cicatrização das úlceras venosas. Além de favorecer o debridamento, o mel não é aderente, é fácil de aplicar e remover, e é de fácil aceitação por parte dos usuários. Os resultados descritos geraram uma linha investigativa no tratamento de feridas crônicas. .

MÉTODO: estudio cuantitativo descriptivo longitudinal. Durante el período de un año se evaluaron 18 pacientes diagnosticados clínicamente de úlcera venosa en sus diferentes estadios, de ambos sexos, adultos, con 13 meses promedio de antigüedad de la lesión. Se realizó la aplicación tópica diaria de miel de Ulmo con curación según la norma técnica de curaciones avanzadas, combinada con el consumo oral diario de 500 mg de ácido ascórbico. El instrumento de seguimiento es la tabla de valoración de úlceras venosas. RESULTADOS: se logró la cicatrización total en el 100% de las úlceras venosas. No se observaron signos de complicación, tales como alergias o infección. CONCLUSIÓN: el tratamiento propuesto mostró excelentes resultados clínicos en la cicatrización de úlceras venosas, presentando la miel propiedades debridantes, no adherentes, fácil de aplicar, remover y aceptación del usuario. Los resultados descritos generaron una línea investigativa en el tratamiento de heridas crónicas. .
